SELF ASSESSMENT QUESTIONNAIRE, WEEKLY ASSESSMENT CHART & EXPLANATION VIDEO
As part of the Initial Consultation process with every new client we fill in the questionnaire below and plot the score on the chart below.
The total score will range between 0 and 70. Quite simply, the lower the score the less control we have over our thoughts, feelings and emotions and the worse we feel. The higher the score the greater control we have over our thoughts feelings and emotions allowing us to feel confident, successful and happy!
As a general guide, I see very few new clients with an initial score lower than 10 as these individuals may not be functioning well enough to even contact me and most new clients will have an initial score below 50.
Underneath each form you will see a link to Download File. If you have a printer it will be a great help to print the forms off. If not, please read the forms and write the answers on a piece of paper.
